The reading log I used both in class and at home. This was stapled in students reading folders next to their strategies to habit chart.

This strategies to habit chart was used in students reading folder. I used sticky notes to track skills that were discussed during conferences. Once students had "mastered the skill", it would be moved to the habit side of the chart.
